After Yuji and Higuruma's fight which saw both characters' morality examined and challenged, it's now Megumi's turn to face the same test. This seems to be an underlying theme of the Culling Game and that makes perfect sense. Being in a kill or be killed situation is nothing new to the cast of Jujutsu Kaisen, but they're used to fighting against Cursed Spirits. It's a completely different game when all their opponents are humans.
Yuji would prefer to get the points they need without killing any humans and Megumi knows that. The only way the fight against Higuruma could have ended on a better note for Yuji is if the lawyer had agreed to stay with him. Megumi, on the other hand, is not completely on the same page as Yuji when it comes to killing his opponents. He's willing to go as far as he needs to in order to get points.
At the very end of the previous chapter, Megumi was shocked to suddenly see an eyeball falling from above outside the building he's in. As it turns out, the eye belonged to Iori Hazenoki, one of Reggie's allies, and it explodes like a bomb. It seems this ally's Cursed Technique involves removing certain body parts and using them as bombs. He's also seen using his teeth in the same way and the parts grow back.
Megumi spends the chapter fighting against Iori and Chizuru Hari (another of Reggie's allies) and ends up killing the latter. These fights are death matches so of course, no one can blame him for killing an enemy who's trying to kill him. However, it is a bit concerning that he genuinely doesn't seem to mind killing humans. In that regard, he's actually like his father, Toji Fushiguro, who was known as the Sorcerer Killer. There is a noteworthy difference between the two, though. Megumi may not mind killing humans if he has to, but he certainly doesn't show any signs of enjoying it. Toji was clearly proud of himself when he killed.
Throughout the chapter, as Megumi is fighting, he curiously protects Reggie's ally Remi from harm multiple times and tries to get her to flee. Despite how angry he is at her for taking him to Reggie, he shows no interest in killing her. Not even when she attacks him after he protects her. This could be considered proof that he won't kill just anyone. But it begs the question, what is it about Remi that gives Megumi pause?
It's possible that Megumi isn't comfortable killing a girl due to his relationship with his sister. It's clear he doesn't mind hurting her in a fight, but killing her is a different story. It could also be because she's weak as he pointed out and he probably pities her despite himself. Megumi knows Remi is no threat to him and would have likely been killed already if she didn't cling to someone like Reggie for protection. This is why he figures that she's most likely disposable to Reggie for that very same reason. Moreover, it's not as if she chose to be a player in the Culling Game.
Megumi attempts to get her to run away by showing her that Reggie and his other allies don't care about her. She desperately demands that he tell her that he'll protect her the way Reggie did. This irritates Megumi as he can't stand people like her who cling to others' words rather than their actions. He sees them as trash.
Interestingly, Reggie has yet to engage in fighting himself during this entire battle. This means it's still a mystery what his Cursed Technique is. It would be problematic if he's able to use Domain Expansion as Higuruma can. Megumi is already struggling to keep up as he's facing multiple people and there's no telling if Reggie has more allies waiting to pop up. Fortunately, Fumihiko Takaba, the comedian sorcerer introduced in Chapter 146 randomly shows up to level the playing field as the chapter ends.
